AN IMMUNOHISTOCHEMICAL STUDY FOR CYTOKERATIN AND VIMENTIN IN SALIVARY GLAND TUMORS

Abstract

Immunohistochemistry is used to determine the origin of tumor cells in paraffin sections
of tumor, and it is useful to diagnose problem cases. Recently, it is reported that a few
tumors has the characteristics of co-expression of epithelial and mesenchymal cell
markers. The co-expression of intermediate filament is important in differential diagnosis
of tumor.
The determine the coexpression pattern and apply it for differential diagnosis,
immunohistochemistry for cytokeratin and vimentin was performed in the paraffin
sections of salivary gland tumors.
The results were as follows
1. In pleomorphic adenoma, expression of cytokeratin was identified in the inner layer of
the tubular structure, squamous metaplastic cells, cellular clusters, stellate shape cells
and plasmacytoid cells and that of vimentin was noted in the outer layer of the tubular
structure, chondroid and meseachymal tissue.
2. In mucoepidermoid tumors, cytokeratin was expressed in the epithelial cells of the
tubular structure and cellular clusters, but not in the mucus-secreting cells ; vimentin
was expressed in the stromal cells but not in the tumor cells.
3. In adenoid cystic carcinomas, cytokeratin was expression the inner layer and vimentin
in the outer layer of the cribriform and trabecular pattern Co-expression of cytokeratin
and vimentin was noted in a few cells of basaloid pattern.
4. In monomorphic adenoma, cytokeratin was expressed in the inner layer of canalicular
structure, but vimentin was not. In basaloid types, cytokeratin was expressed in some
tumor cells but vimentin was not.
5. Carcinomas arising from pleomorphic adenoma showed similar immunohistochemical
findings to pleomorphic adenoma except strong expression of cytokeratin in the
dysplastic cells.
Form the above co-expression of cytokeratin and vimentin was identified in pleomorphic
adenoma and adenoid cystic cacinoma and it is suggested to be used in differential
diagnosis of the salivary gland tumors.
